New Patient Registration

registration1The doctors welcome new patients who live within our practice area. 

Please click for a map of the area our surgery covers

Please ask for a Registration Form and a New Patient Questionnaire at the Reception desk. Once you have attended a new patient medical, you will be registered with the practice.

It is important that you attend for a new patient medical appointment to enable the health care assistants to provide the GPs with a general medical summary

When you attend for your new patient appointment please bring with you the right hand side of the repeat prescription from your previous GP.  

Please ensure that you have one month supply of medication from your previous GP.  

Please contact reception for further information.
